How to Apply

IN ORDER TO BE ELIGIBLE FOR TREATMENT AT RAINBOW RECOVERY CENTER, AN APPLICANT MUST MEET CERTAIN REQUIREMENTS:

  • You must be at least 18 years of age.
  • You must have at least 1 year of opiate addiction.
  • You must provide verification of current physical dependency.
  • you must sign a consent in writing allowing you to voluntary participate in methadone treatment.
  • You must be able to verify pregnancy (if applicable).

In addition to this, when you come to the center for your intake, you MUST bring a form of identification such as driver’s license, state photo issued I.D., or a passport, and you MUST bring proof of your insurance such as your MA card. IF YOU ARE SELF-PAY, you must bring proof of being able to pay for services, such as a bank statement, paycheck stubs, or if somebody is paying for you, THEIR proof of income.

*Federal regulations state that priority for access to treatment will be given to applicants who are pregnant or have tested positive for HIV.

MEDICAL REQUIREMENTS:
Rainbow Recovery Center requires a complete medical profile once you’ve finished the screening and intake procedures. This profile includes the following areas from you: medical history, physical examination, a urine sample, and an RPR test for Syphilis. Our medical staff will also explain about what methadone is, how it works, and answer any questions that you have about methadone. A referral will be provided for HIV testing at your request. If you are a pregnant woman seeking treatment with us you will be provided with referrals  for prenatal care immediately and strongly encouraged to follow through. The medical staff will want to meet with you more frequently to monitor your progress and ensure stabilization on methadone.
